Whilst DoCoMo use WCDMA 3G, only Softbank use GSM 2G in Japan so any Japanese iPhone could be 3G only if it was on DoCoMo, unless Apple invested a lot to support a legacy 2G technology as well just for call fallback when 3G reception gets bad. This does actually matter as whilst japan is drenched in more 3G than any other country, 3G signals do struggle to get through walls and without 2G fallback it wouldn't be a good voice handset.
KDDI, the other network, are pure CDMA family so they are also out without a lot of extra effort – though that extra effort would at least allow Verizon in the US a chance of getting the handset, if that were contractually possible.
Really, Apple are unlikely to want to start branching into multiple network technologies, they will stick with GSM as it's cheaper and works almost everywhere. Anyone who understood this basic point wouldn't ever have been expecting DoCoMo to get an iPhone ;)
